Abstract: The article deals with the cost-effectiveness analysis of innovative projects in the sphere of biomedicine. The analysis of these projects makes it possible to estimate the economic benefits of the projects and will contribute to the commercialization of intellectual property created in our region. The development strategy of medical science in the Russian Federation until 2025 calls for a cluster principle of organization. It is proposed to use a public-private partnership in the form of concession as a model for cluster creation. The selection of innovative biomedical projects should be carried out using their cost-effectiveness analysis. It is noted that the use of real options is a way to quantify the project management flexibility
